Home Health Registered Nurse (RN) – Part-Time

Flexible Schedule | Part-Time Day Hours

Territory: Bellingham, MA, and surrounding communities

What You’ll Do

As a Home Health RN, you will manage an assigned group of patients and provide skilled, personalized care throughout their healthcare journey. Responsibilities include: - Completing comprehensive nursing assessments - Developing and updating individualized plans of care - Performing admissions, routine visits, recertifications, and discharges - Monitoring patient progress and identifying changes in condition - Coordinating care with physicians, caregivers, and interdisciplinary team members - Educating patients and families about medications, disease management, and self-care - Helping patients remain safe and independent at home - Providing proactive interventions that help prevent avoidable hospitalizations - Completing accurate and timely clinical documentation

What You’ll Need - Current, unrestricted Massachusetts Registered Nurse license - At least one year of nursing experience in a clinical care setting - Home health or home care experience is highly preferred - Consistent availability for at least one full 8-hour day each week - Valid driver’s license, reliable transportation, and current automobile insurance - Ability and willingness to travel throughout the assigned territory - Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds
