We care for patients in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs). We partner with medical teams to provide comprehensive, patient-centered care focusing on restoring function, managing pain, improving mobility.

Ideal Candidate - A Nurse Practitioner/Physician Assistant seeking to join the Chestnut team on a part-time or 1099 contract basis - 8-10 hours available per week, flexible day/time - Interested in physical medicine and rehabilitation (PM&R)
What You’ll Do - Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions. - Collaborate with therapists and the facility’s medical team on treatment plans. - Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
Requirements
Requirements - Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice - If an NP: ANCC or AANP certification - Current DEA and state Controlled Substance License (required upon start date) - SNF, pain management, or physiatry experience a plus
