This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care. While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings. font-size: 10pt;" data-section-id="5og9mo" data-start="281" data-end="516">Support day-to-day clinical workflows by assisting with referrals, laboratory orders and follow-up, medication refill requests, durable medical equipment (DME) coordination, and other care coordination tasks</li> <li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;" data-section-id="kdsgp4" data-start="517" data-end="696">Maintain frequent communication with patients, providers, families, and interdisciplinary teams to address patient needs, resolve barriers to care, and ensure timely follow-up</li> <li style="font-family: helvetica, arial, sans-serif;
